Enrollment
Before joining FORTE, all potential attendees participate in a friendly, informal assessment conducted by the Operating Director and the FORTE Program Supervisor. This relaxed meeting is a chance to get to know each other, discuss success plans and goals, and ensure the program is a great fit. Attendees are encouraged to bring their guardian and anyone else they’d like to involve. We’re happy to meet at the attendee’s residence, the Forte office, or a convenient community location—whatever works best!
If accepted into the program, attendees will receive a preliminary Individual Support Plan (ISP) and Behavioral Support Plan (BSP) based on the information shared during the assessment. After 30 to 45 days, the attendee, their guardian, the Operating Director, and Leadership team will meet again to review and update these plans to better reflect the attendee’s progress and needs.

FORTE Day Program
FORTE is a supportive and engaging day program designed for high-needs behavioral adults. We focus on providing meaningful activities both in the community and within our well-equipped building. Our program offers a structured and nurturing environment where participants can thrive socially, emotionally, and developmentally.
Program Highlights:
Assessment Process: Each prospective participant undergoes an assessment to ensure they meet the required acuity level and can benefit most from our specialized services.
Community Involvement: Regular outings and activities that promote social engagement and community integration.
Opportunities for Growth: Clients are encouraged to assist with tasks such as assembly, cleaning, cooking, office duties, stocking, and more. Compensation may be available for their contributions.
On-Site Amenities:
Games, movie sessions, and a variety of recreational activities
Sensory room for calming and therapeutic experiences
Animal therapy for relaxation and emotional well-being
Kitchenette with an oven for cooking activities
Snacks and a nutritious lunch provided daily
Personal Care Support: Full bathroom with a walk-in shower available for staff-assisted showers when needed
Dedicated Staffing: Personalized support is available based on individual participant needs
